Our team works alongside individuals and families navigating complex medical, mental health, and social systems by offering personalized guidance, resource connection, and emotionally intelligent advocacy.

Residents Navigating Care without Insurance or Stable Income
Navigation of Medicaid and public benefits (SNAP, SSI, Housing vouchers, etc.)
Connection to sliding-scale or free clinics (Dental, and mental health services)
Assistance with medical home establishment to reduce Emergency Department visits
Transportation coordination for appointment and pharmacy access
